Our facility constantly had issues revolving around discharging patients to area nursing homes resulting in an additional 1-2 day stay which were denied days. We assembled a lean team led by Power 2 Transform which focused on identifying ways to facilitate the discharge.  Numerous problems were identified during the process, as well as workable solutions.  Having a multidisciplinary team of hospital staff (case management, nurses, paramedics, medical records), as well as representation from all area nursing homes helped greatly in accomplishing our goals.  We identified savings of $90,000 based on our initial lean results.  The team will continue to meet quarterly to keep our finger on the pulse and generate more savings.
